Does an Impacted Wisdom Tooth Need Removal?

Impaction describes position, not the decision

A wisdom tooth is impacted when it cannot erupt into a normal functional position. Some impacted teeth can be monitored; others cause disease or damage that makes removal appropriate. A recommendation should identify the problem being treated rather than rely only on the word “impacted.”

Reasons to assess removal

Repeated inflammation around a partly erupted tooth, unrestorable decay, damage to the neighbouring tooth or another associated lesion may justify treatment. The examination considers symptoms, cleaning access and the health of both teeth. Preventing future crowding alone is not a sufficient general explanation for removing every wisdom tooth.

Planning around nearby structures

For lower wisdom teeth, the relationship to nerves matters. For upper teeth, the sinus and surrounding anatomy may be relevant. Ordinary dental radiographs may answer the question; additional CBCT is considered when it is likely to change management. In selected lower-tooth cases close to a nerve, a specialist may discuss coronectomy, which leaves roots intentionally, rather than complete removal.

What recovery can involve

Soreness, swelling and restricted opening can occur after surgery. The expected course depends on difficulty and individual factors. Risks include infection, dry socket, bleeding and altered sensation; discuss the risks specific to your tooth before consenting.

Follow the extraction instructions provided by your surgeon. Increasing pain after initial improvement, persistent bleeding, worsening swelling or fever needs advice. Difficulty breathing or swallowing requires urgent emergency assessment.

Questions worth asking

Why is removal preferable to observation in my case? What are the risks of leaving it? How close are important structures? What should I expect during recovery? A careful answer should explain both the proposed procedure and the reasonable alternatives.
